Physiopathology of the TMJ:

what the prosthodontist has to know

Dr. Daniele Manfredini

Moderator: Dr. Stefano Gracis

The field of temporomandibular disorders (TMD) is a field where the clinician is confronted with a mass of confusing and controversial information, often not supported by any evidence, full of dogmas debunked, but not yet completely buried! The goal of this day is to clarify certain aspects of fundamental importance for the prosthodontist and then bring our knowledge of TMD in line with the new evidence to be able to implement treatment strategies that work.

SOME OF THE QUESTIONS POSED TO THE SPEAKER

  • What role does the dental occlusion have in TMD?
  • Does bruxism cause TMD?
  • On what is based the diagnosis of TMD?
  • Can a prosthetic rehabilitation (i.e., occlusal reorganization) be a treatment for TMD?
  • Do patients with TMD benefit from surgical treatment?
  • For proper prosthetic treatment of patients with TMD, do you have to recapture the articular disc?
  • Do the occlusal splints work because they reposition the jaw in an ideal position?

 TOPICS COVERED

  • Anatomy of the temporomandibular joint
  • The role of dental occlusion in the etiology of temporomandibular disorders (TMD): how and why abandon the legacies of the past
  • The pathophysiology of TMD today: the theory of overload
  • Bruxism
  • The diagnosis of TMD
  • Basic concepts of TMD management
  • The occlusal splints, between erroneous paradigms and paradoxical effects
  • The prosthetic treatment in patients with bruxism and TMD

QUESTIONS FOR THE SPEAKERS

  • Which are the technical differences among the various intraoral digital systems and how do they influence the accuracy of the digital data?
  • Which are the technical differences among the various laboratory digital scanning systems and how do they influence the marginal precision of the restorations?
  • Are intraoral scanners indicated for rehabilitations that involve several natural teeth abutments or implant abutments that have to be splinted together?
  • Besides the scanner, which other factors influence the marginal quality of a cad-cam restoration?
  • How do milling units differ?
  • Which are the differences between a 4-axis and a 5 axis milling unit for what concerns the restoration’s quality, marginal precision, materials that can be milled, and speed of milling?
  • What is your opinion about “chairside” digital systems?
  • What consequences will the transition from analogue to digital dentistry have on the dental laboratory and the dental technician? How does his/her role change?
